THOROLD MINOR BASEBALL - INDOOR BASEBALL TRAINING LIABILITY WAIVER

PLEASE READ CAREFULLY

I acknowledge and understand that participating in indoor baseball training organized by Thorold Minor Baseball (hereinafter referred to as "TMBA") involves inherent risks and dangers. In consideration for being allowed to participate in the indoor baseball training, I agree to the following terms and conditions:

1. Assumption of Risk: I am aware that participating in indoor baseball training activities includes the risk of injury or death, and I willingly assume these risks on behalf of myself and my child.

2. Health and Medical Conditions: I hereby represent that my child is in good health and has no physical condition that would prevent them from participating in indoor baseball training. I agree to inform TMBA of any pre-existing health conditions or allergies that my child may have.

3. Supervision: I understand that TMBA will make reasonable efforts to ensure the safety and supervision of participants during indoor baseball training. However, I acknowledge that it is not possible to eliminate all risks, and accidents or injuries may still occur.

4. Release of Liability: I hereby release and discharge TMBA, its officers, directors, coaches, volunteers, and any facility used for indoor baseball training from any and all claims, demands, actions, or causes of action, whether in law or in equity, arising out of any injury, illness, or damage that may occur as a result of my child's participation in indoor baseball training.

5. Medical Treatment Authorization: In the event of an emergency, I authorize TMBA and its representatives to obtain medical treatment for my child, including transportation to a medical facility, if deemed necessary. I understand that I will be responsible for any medical expenses incurred.

6. Photography and Publicity: I grant TMBA the right to use photographs or videos of my child taken during indoor baseball training for promotional or educational purposes without compensation.

7. Compliance with Rules: I agree that my child will comply with all rules and instructions provided by TMBA during indoor baseball training.
